TERMS AND CONDITIONS
This offer is based on an initial assessment that I met CommonWealth One Federal Credit Union’s (herein referred to as Credit Union) initial criteria for creditworthiness. By clicking yes below and submitting this offer to the “Credit Union,” I certify that I have read this new offer, and the information I have provided is true and correct to the best of my knowledge. I certify that I am at least 21 years of age or older and a United States citizen or a permanent resident alien of the United States. I also authorize Credit Union to review my credit and employment history and any other information afforded to Credit Union by law in order to process my membership application and acceptance certificate. I understand that the credit line on this account will be determined after a review of my acceptance certificate by Credit Union and will be based on various factors, including income. The minimum approved credit line is $1,500. Credit Union maintains the right not to open this account if: a) the information provided is incomplete, inaccurate or cannot be verified, or if I no longer meet Credit Union’s standards for creditworthiness; b) my name and/or mailing address have been altered; c) Credit Union receives my response after the offer has expired; d) my income reported on this response certificate is insufficient to support the opening of this account; or e) you do not meet the Credit Union's criteria for membership. I understand I have the right to review my credit history by contacting the credit reporting agencies.

We may change the APRs, fees and other terms of your account at any time in accordance with applicable law and the Account Agreement, which we will send you when your account is opened. Factors we may consider in determining whether and how to change your terms include the frequency and severity of defaults and other indications of risk on accounts with Credit Union and other creditors. To the extent allowed by law, the new terms will affect all outstanding balances.

This offer is nontransferable and is void to residents of GU, PR, VI and all other U.S. dependent areas. The terms of this offer may not be applied to existing Credit Union accounts.

California Residents: Applicants: 1) may, after credit approval, use the credit card account up to its credit limit; 2) may be liable for amounts extended under the
plan to any joint applicant. As required by law, you are hereby notified that a negative credit report reflecting on your credit record may be submitted to a credit reporting
agency if you fail to fulfill the terms of your credit obligations. Married applicants may apply for credit separately.

New York and Vermont Residents: Credit Union may obtain at any time my credit reports for any legitimate purpose associated with the account or application.
On my request, I will be informed if such a report was ordered and I will be given the name and address of the agency furnishing the report.

Ohio Residents: Ohio anti-discrimination laws require creditors to make credit equally available to all creditworthy members and that credit reporting agencies
maintain separate credit histories on individuals upon request.

Massachusetts Residents: You have the right to prohibit the use of information about you contained in your file with any consumer-reporting agency in
connection with any credit transactions that you have not initiated. You can exercise this right by contacting consumer-reporting agencies through their toll-free
notification systems established for this purpose.

Married Wisconsin Residents: Wisconsin law provides that no agreement, court order or individual statement applying to marital property will adversely affect
a creditor’s interest unless prior to the time credit is granted, the creditor is furnished with a copy of the agreement, statement or court order, or has actual
knowledge of the provision.

If I am applying for a Balance Transfer, I authorize Credit Union to charge my approved Credit Union credit card account for the amount(s) listed for transfers. I understand that I am applying for a transfer amount up to the total amount listed on my Balance Transfer. Balance transfers are processed in the order listed and for the amount requested, or up to my available credit line. Credit Union will not be responsible for any charges, fees or transactions billed to me by the accounts listed on the balance transfer form. I understand Credit Union will advise me if it is unable to process my payment request for any reason. I will continue to make payments until the balance transfer is complete. I may not transfer balances to pay off any existing Credit Union accounts. I agree to abide by the complete Credit Union terms and conditions on this account that will accompany my card when delivered.

I also understand that membership in CommonWealth One Federal Credit Union is required upon acceptance of this offer.  I understand membership in the credit union is free and open to residents of Virginia and the surrounding states, as well as members of approved Select Employee Groups.
